How we helped care managers share care plans with patients and providers.

 

Background

Current State

To integrate the care plan into our Population Health product thoroughly, we had to also make updates to the way it was shared. I identified simple changes we could make to the experience that would have a big impact on our users.

We could speed up the time it took for care managers to share a care plan by streamlining all the CTA’s and leveraging pre-selected content for the right users.

FIND PRINT BUTTONS
There were five CTA’s that all meant “share” but had different names in various locations.

Each location had its own set of pre-selected content with additional content based on the location of the print button, yet no single place to find it all.

DOCUMENT & RUN QUERY
I documented each CTA with it’s pre-selected and available content 

My team queried our users to determine which CTA’s were most commonly used as well as which options were changed

REORGANIZE BASED ON USER TYPE
I then used our findings to reorganize the options based on recipient of the shared care plan instead of the location of the CTA

 
 

 
 

Organized

One Location

Instead of maintaining 5 CTA’s in different locations, we created a central place where users could go to share care plan information at all times.

We organized pre-selected content around three different recipients of care plans - care managers, patients, and quality managers. 

We exposed all the content so that care managers didn’t have to “find Waldo” behind different CTA’s when they were looking to share a bit of information.

Last but not least, we gave our users the proper space necessary to interact with and preview the content they wanted to share.

Release of Shared Care Plan

This experience went live for our Population Health product in July 2018 and has received positive feedback from our users. I am now working on ways to share the care plan with both clinicians and patients in our Clinicals Ambulatory product which has many more ways of sharing today, all of which were created in silos.

 
 
 
Aside from all her skills, something that I really appreciate is that [Jenn] brings a new perspective to our team. Noreen, Chelsey, Luke, and I have been working on Pop for years now, and sometimes that creates a blind spot and bias for us when we look at the product.
